Trial Identifier

NCT06952803

Phase

III

Regimen

Saruparib + physician’s choice ADT; placebo + physician’s choice ADT; saruparib + physician’s choice ADT + abiraterone (and prednisone/prednisolone); placebo + physician’s choice ADT + abiraterone (and prednisone/prednisolone)

Study Design

Inclusion criteria

The key inclusion criteria are –

  • Male patients with histologically confirmed prostate adenocarcinoma.
  • Newly diagnosed high-risk/very high-risk localized/locally advanced prostate cancer or high-risk biochemical recurrence (BCR) post-radical prostatectomy.
  • Submission of FFPE tumor tissue sample and confirmed BRCA1/BRCA2 mutation status.
  • Patients must have post-radiotherapy (RT) CT/MRI and bone scan confirming no disease or disease confined to the pelvis (M0).
  • ECOG performance status of 0 or 1, stable for 2 weeks prior to randomization.
  • Minimum life expectancy of 12 months and adequate organ and bone marrow function per protocol.
  • Patients should have received primary or salvage RT with curative intent, eligible for randomization within 10 months of diagnosis. Metastases-directed RT outside the pelvis is allowed.
  • All patients will have received planned ADT regimen with GnRH analogue.
  • Patients must not father children or donate sperm during the study and for 6 months post-intervention. Condom use (with spermicide, if permitted) required during this period.

Exclusion criteria

The key exclusion criteria are –

  • Patients with history of MDS/AML or features suggestive of MDS/AML.
  • Predisposition to bleeding (eg, active peptic ulcer, recent hemorrhagic stroke, proliferative diabetic retinopathy).
  • Severe cytopenia persisting >2 weeks.
  • Refractory nausea/vomiting, chronic GI diseases, inability to swallow, or prior bowel resection affecting drug absorption.
  • History of another primary malignancy (with exceptions).
  • Persistent CTCAE Grade ≥2 toxicities from prior anticancer therapy and cardiac issues (eg, arrhythmia, cardiovascular disease).
  • Active, uncontrolled hepatitis B/C, HIV, or tuberculosis.
  • Any prior chemotherapy (ie, docetaxel) or immunotherapy; any prior treatment with a poly (ADP-ribose) polymerase (PARP) inhibitor.
  • Prior treatment within 14 days with blood product support or growth factor support.
  • Concomitant use of strong inducers and inhibitors of CYP3A4 (applies to saruparib and abiraterone) or herbal supplements within 21 days or at least 5 half-lives (whichever is longer) of randomization.
  • Concomitant use of  QT-prolonging drugs with TdP risk.
  • Hypersensitivity to saruparib or its excipients.

Target Overview

Poly [ADP-ribose] polymerases (PARPs) are a group of proteins known to be involved in the repair of single-strand breaks in DNA.1

A healthy cell’s DNA undergoes damage every day, with approximately 10,000 spontaneous single-strand breaks that require repair.1

Inhibition of PARP1 with AZD5305 leads to the trapping of PARP bound to DNA single-strand breaks, stalling of replication forks, their collapse and the generation of DNA double-strand breaks. In tumor cells that already have compromised DNA repair mechanisms (such as loss of BRCA1 or BRCA2 function, affecting double-strand break repair), inhibition of PARP leads to excessive accumulation of DNA damage and may cause death of the cell.1,2

Compound Overview

Saruparib (AZD5305) is an investigational, new generation PARP inhibitor. It is designed to selectively inhibit and trap PARP1, enabling efficacy, while broadening the opportunity for clinical benefit by allowing combination approaches with different treatment options.3,4
